The Chronic Illness Relief program provides one off funding. The program does not provide ongoing case management.

Eligibility

The Chronic Illness Relief program's target group is recognised as being very broad.

People with Arthritis, Cystic Fibrosis, Diabetes, Epilepsy, Motor Neurone, Multiple Sclerosis, Muscular Dystrophy, Parkinson's, Thalassemia, allergies, cancer, HIV/AIDS, haemophilia & chemical sensitivity are considered a priority.

People on waiting lists for services and people receiving services but whose needs could best be met by a "top up" of funding, are also considered a priority.

How do I Access the Service?

To make a referral send the Department of Human Services Service Coordination (Consumer Information, Consumer Consent and Summary and Referral Information) forms to a Team Leader. These forms are available on www.connectingcare.com or by calling our office.
