The Home Assessment and Rehabilitation Team is a health care team which provides assessment and therapy services to people in their homes.
Home Assessment and Rehabilitation Team works in the Greater Bendigo area with carers and people who are frail aged or disabled and who need home-based services.
Each client participates in developing a care plan to meet their needs. This is a plan of action to deal with any problems identified during the assessment.
Assessment services include:
Aged Care Assessment Services (ACAS):
- Aged care assessments are carried out to establish accommodation and care needs of frail aged individuals
Access to Home Care Services:
- The Home Assessment and Rehabilitation Team works with the City of Greater Bendigo to carry out the assessments for home care services
Specialised Assessments:
- Medical & Nutritional
Functional Assessments of:
- Daily living tasks
- Mobility/balance
- Personal safety/home environment
- Memory
- Social situation
- Medication management
Assessments for eligibility for:
- care accommodation
- Respite care
- Funding for equipment, home modifications and services
- Personal alarm systems
- Community Aged Care Packages (CACP)
- Geriatric Evaluation and Management at Home (GEM)
- Carers allowance
Clients can be referred to the service via:
- General practitioners
- Hospitals
- Health care providers
- Self
- Carers, relatives, friends
