The Transition Care program provides short-term support and management for older people after a stay in hospital. Participants are provided with a care package of services as part of an ongoing but slower recovery process while options for their longer term care arrangements are considered.
There are fifteen places available in the Transition Care program in Bendigo:
- Seven community (home-based) places for people who reside within a radius of 10 kilometres of Bendigo CBD
- Eight bed based places at Bentleys Aged Care supported accommodation residence (three of these places are flexible and may be used as either bed or community places)
Eligibility
To be eligible for the Transition Care program you must be:-
- At least 60 years of age
- Medically stable and ready for discharge from hospital
- Eligible to enter low or high care permanent residential care
- Assessed by the Aged Care Assessment Service as suitable for the Transition Care Program
Mix of services
Depending on your assessed level of need, the Transition Care program will offer a package of services tailored to your needs that may include:-
- Nursing support
- Low intensity therapy or rehabilitation (physiotherapy, occupational therapy)
- Personal care, home care, respite, meal delivery
- Medical support such as GP oversight and geriatrician support
- Social work and case management
Fees:
Daily fee for home based and bed based service as negotiated with Transition Care program co-ordinator
