Transport Home

Transport home or to the patient's usual place of residence (such as a hostel or nursing home) is the responsibility of the patient or the patient's family or carer. Please ensure arrangements have been made prior to discharge.  Please discuss any concerns or issues regarding transport arrangements with the patient's nurse.

Transport home by the ambulance service is only arranged in cases of medical need as determined by nursing and medical staff and only for ambulance subscribers and pensioners.
