Case Management Services

Bendigo Health Case Management Services provide support for people with a disability, chronic illness or age related frailty who:

  • Have complex needs affecting their independence and safety
  • Want to remain living at home in their local community
  • Has a carer who needs support in order to maintain their caring role.

We provide a case manager to work with people. The case manager becomes the main contact person. They work in partnership with people and their family to identify the support options that best suit people’s needs, within the funding available. 

 

Who can ask for help?
You can ask for help if you are:

  • Over 65 and need a range of different types of care or
  • You are under 65 and living with a disability or
  • You can ask for help if you are over 45 years and from an Aboriginal community.

What services are available?
Some of the services available include:

  • Personal care - bathing, showering or personal hygiene
  • Home cleaning
  • Respite care
  • Social support
  • Equipment
  • Meal preparation
  • Garden maintenance.

Making a referral
To make a referral send the Service Co-ordination Tool Template (SCTT) to one of our offices as detailed below.

Required forms are:

  • Confidential referral cover sheet
  • Consumer information
  • Consumer consent and
  • Summary and referral information

These forms are available on:
www.connectingcare.com or by calling any of our Case Management Services offices.


For people living in the Greater City of Bendigo Council area and Shire of Campaspe all referrals must be sent to the Bendigo Health Referral Centre.

Our normal business hours are Monday to Friday, 8.30am to 5.00pm


Waiting list
There may be a waiting period depending upon the demand and availability for packages. Once referrals are processed, they will be prioritised and the referrer notified of the outcome.

Fees
A contribution toward the cost of the services provided is requested for some of our packages
