Post Acute Care (Loddon Mallee)
The Post Acute Care (PAC) program provides individually tailored packages of care to patients needing additional support with their recuperation following discharge from hospital.


Post Acute Care is a time limited, short term intervention, designed to assist patients to recuperate following a hospital admission and to help their independence or transition to other care where required.

 

Post Acute Care projects have a flexible pool of funds from which to purchase and co-ordinate community services to support these patients, such as home nursing, home help, personal care, child care and allied health services.

The Post Acute Care co-ordinator will work closely with the client and/or carer and health care workers to ensure that the most appropriate services are provided by the service.

Who is Eligible?

Patients need to be assessed as requiring additional services to assist with recuperation or transition to continuing (other) care following a stay in hospital. This includes patients who have presented to our Emergency Department, Oncology and Radiotherapy departments.

Services typically provided by Post Acute Care:

  • Home help
  • Personal care
  • Respite
  • Transport
  • Allied health - physiotherapy, occupational therapy etc.
  • Case management
  • Nursing care
