What the clinic does
Service for clients with neurological conditions eg following a CVA, an ABI or neurosurgery. The team also manages clients with progressive neurological disorders such as MS, motor neurone disease or Parkinson’s diseases and offers: • Fitness and strengthening programs • Prescription of aids and equipment • Individual and group counselling from psychology • Neuropsychology assessment • Social/leisure skills retraining/vocational retraining • Driving assessment • Liaison with community services.

How to refer your patient
You can provide information using the Bendigo Health SCTT form, your personal letterhead or a computer generated form.
Information about the patient’s condition, functional ability, the impact their condition is having on their normal daily activities, some indication of the expected outcomes would help us in determining how best we can meet the needs of your patient.
You may fax the referral in and then post the original.
