How to refer your patient  

You can provide information using the Bendigo Health SCTT form, GP personal letterhead or a computer generated form.

Information about the patient’s condition, functional ability, the impact their condition is having on their normal daily activities would help us in determining how best we can meet the needs of your patient.

Please include relevant past history and current medications and pathology results with your referral

You may fax the referral in and then post the original.

Rapid access is available according to need.
