Flexible Care Packages, Case Management Services

What the service does

Flexible support packages offer support to individuals seeking short-term assistance (approximately three months) and people requiring intensive or longer term support.

Case managers work in partnership with individuals and their families and carers to identify support needs and develop a plan to increase the individual's capacity and family resilience.

Flexible support packages are designed for children, young people and adults with a disability, as well as their families and carers where relevant.

Packages are allocated according to the relative need of people and consideration of the following:

  • Ageing carers
  • Carers with fragile health
  • People who require brief and timely intervention to prevent or minimise the need for long-term, high cost support
  • People who have high or complex support needs
  • People who experience significant difficulties accessing community services and flexible service responses
  • People who demonstrate significant levels of stress and where the family is at risk of breaking down
  • People whose support needs change rapidly and are at risk of moving to supported residential services.
