How to refer your patient
For urgent referrals contact clinic staff direct via 5454 8031.
You can provide information using the Bendigo Health SCTT form, GP personal letterhead or a computer generated form.
You must also fill out the ‘initiation of insulin therapy and stabilisation in an ambulatory setting’ form completely to ensure your patient is seen through this clinic click here for the form (PDF 37 KB).
Information about the patient’s condition, functional ability, the impact their condition is having on their normal daily activities would help us in determining how best we can meet the needs of your patient.
Please include relevant past history and current medications and pathology results with your referral.
You may fax the referral in and then post the original.
