What the clinic does 

Specialist multidisciplinary team catering for children and adolescents, aged between 0-16 years who, as a result of injury, medical/surgical intervention, or functional impairment, would benefit from a program of goal focused and time-limited developmentally appropriate rehabilitation

How to refer your patient

You can provide information using the Bendigo Health SCTT form, your personal letterhead or a computer generated form.

 

Information about the patient’s condition, functional ability, the impact their condition is having on their normal daily activities, some indication of the expected outcomes would help us in determining how best we can meet the needs of your patient.

 

You may fax the referral in and then post the original.
