For people with moderate to severe chronic lung disease or chronic heart failure who are medically stable and have optimal medication management.
The programme aims to return patients to as normal a life as possible and achieve optimal independent function in all life activities.
Education on the impact of chronic lung disease or CHF.
- Fitness, exercise and activities of daily living plans
- Support for patient, carers and families
- Communication with physicians, GPs.

How to refer your patient
|
You can provide information using the Bendigo Health SCTT form, your personal letterhead or a computer generated form.
Information about the patient’s condition, functional ability, the impact their condition is having on their normal daily activities, some indication of the expected outcomes would help us in determining how best we can meet the needs of your patient.
You may fax the referral in and then post the original.
