What the clinic does
Residential inreach is a nurse led program that provides specialist assessment, co-ordination, care provision and treatment for residents within aged care facilitators.
The program provides an alternative to emergency department presentations for the management of acute conditions and aged related health issues.
The team is able to access other services and disciplines according to the needs of the resident.

Eligibility
The client must be a resident of an aged care facility and consent has been obtained from the resident, their family and their GP.

07:00am - 03:15pm Tue, Wed & weekends
07:00am - 07:30pm
Mon, Thur, Fri
Referral process
Referrals can be from the residents in the facilities, their family/friends, facility staff or GP.
