Assessment including functional/workplace assessments Wheelchair assessment, pressure and seating assessment Exercise – gym/hydrotherapy Driving Assessment Home visits, modifications and equipment Information, support and counselling.

How to refer your patient	You can provide information using the Bendigo Health SCTT form, your personal letterhead or a computer generated form. Information about the patient’s condition, functional ability, the impact their condition is having on their normal daily activities, some indication of the expected outcomes would help us in determining how best we can meet the needs of your patient. You may fax the referral in and then post the original.
