Please include all relevant, recent investigation results, medications and the reason for the referral and what further interventions you expect the Emergency Department to perform.

Discharge summaries

The Emergency Department endeavours to return, by fax, a discharge summary with recommended follow-up actions by primary care clinicians.

If you work in more than one practice, please indicate clearly which practice and the fax number you need the discharge summary forwarded to.
