Extended Aged Care at Home

What the service does

The Extended Aged Care at Home (EACH) program enables frail aged people to remain in their homes supported by high level care.

EACH are individually planned and co-ordinated packages of care designed to encourage and help maintain the clients independence in their home.

Case managers work in partnership with clients and their families and carers to identify needs and develop a support plan to increase the client's capacity and family resilience.

EACH packages are very flexible and depend on the complexity of the care needs of the client.

The types of services that may be provided as part of an EACH package include help with nursing care, personal care, social support and home help.

Services provided can change as the client's care needs change.
