What the clinic does	A specialist multidisciplinary early diagnostic team who assists people with memory loss or changes in their thinking and those who support them. The clinic provides: · A comprehensive assessment. Including full medical assessment · Expert clinical diagmosis · Information on treatment, education and support · Assistance with future planning for client and family · Linkage and referral for clients and family to other services · Prior and post clinic consultation with the referring GP for assistance with future primary mangagement.

How to refer your patient	You can provide information using the Bendigo Health SCTT form, your personal letterhead or a computer generated form. Information about the patient’s condition, functional ability, the impact their condition is having on their normal daily activities, some indication of the expected outcomes would help us in determining how best we can meet the needs of your patient. You may fax the referral in and then post the original.
