The Rural Health Team (RHT) is primarily a Home and Community Care (HACC) funded allied health service that supports people to stay at home safely and independently.

To be eligible for Rural Health Team services clients need to be:

• Frail aged, or
• Have a disability, or
• Care for someone who is frail, aged or has a disability and
• Live in the Loddon Mallee region
