What the clinic does

The service involves:

 

1. Supporting health services to improve wound management practices by providing:
- Staff education
- Help with the development of wound
related forms, policies, procedures,
guidelines and resource materials
- Support staff to network and build
clinician support systems.

2. Consultancy relating to specific clients - may be onsite or offsite depending on needs and availability.

Offsite consultancy will involve communication using clinical photography, email and phone communication.

Eligibility

 

Clients from a high care public sector aged residential care service (PSRACS) or an active client of a district nursing service (DNS), where:
• A wound is deteriorating
• The wound has not healed for more than three months
• The staff caring for their wound need specific education in order to deliver the care required
• The staff caring for their wound are having difficulty co-ordinating the care plan across services
• The staff caring for a patient’s wound are experiencing barriers when attempting to implement a care plan based on best practice.
Referrals are NOT accepted from clients who are a community client with wounds being cared for primarily by :
GPs or GP practice nurses
• Low care aged residential care staff
• Other low care residential care staff.

Or who are currently a inpatient in an:
• Acute hospital (public or private)
• Rehabilitation / subacute unit
• Hospital in the Home Program
• Or are currently a resident in a private high care aged residential facility or a Hospital in the Home client.

When referring, please allow around two hours for an initial assessment and as the wound CNS does not see patients alone, ensure a nurse from your facility is free to accompany the wound CNC. 

The aim of the consultation is to build staff capacity, so education ‘in context’ is the major goal.

Please attach any investigation results: blood tests (eg FBE, U&E, LFT, HbA1c), radiology results (eg angiogram, venous and/or arterial duplex ultrasound, x-ray/MRI screen for osteomyelitis).
