To ensure ongoing care, planning for a patient's discharge from hospital begins (for non-emergency patients) before they are admitted and continues during their stay in hospital.
The patient and their family are encouraged to discuss individual needs with nursing staff prior to discharge.
Before the patient leaves hospital, information is provided about medications, pain relief, diet and safe levels of activity. Patient's requiring a medical certificate must ensure this is arranged prior to discharge.
The patient is also advised when they need to see their local doctor.
Discharge is between 9.00 am and 10.00 am (except for Day Procedure patients).
Staff in the Admissions and Bookings office are available to help the patient fill in any forms.
Patients must arrange their own transport home from hospital. A discharge lounge is located on level 1, Stanistreet House where patients can wait for transport home in a safe and comfortable area.
Emergency patients are discharged from the Emergency Department and given appropriate information prior to discharge.
