Support at Home

Support at Home package management is delivered through Community Care Services, providing care coordination that helps people live safely and independently at home while remaining active members of their community.

What we do

Bendigo Health Community Care Services provide quality case management and care coordination services to improve people’s quality of life and independence at home.

Some of the services we can support clients to access include:

  • Personal care - bathing, showering or personal hygiene
  • Allied Health and Nursing
  • Domestic Assistance
  • Respite care
  • Social support
  • Assistive Technology
  • Home Modification
  • Meal preparation
  • Garden maintenance

Care Partners will work with you to identify the personalised support and services you need to achieve maximum independence and quality of life.

Our service is available to people living across the Loddon Mallee with local staff across the local government regions. We offer services in the local government areas of:

  • Greater Bendigo
  • Buloke
  • Campaspe
  • Central Goldfields
  • Gannawarra
  • Loddon
  • Macedon Ranges
  • Mount Alexander
  • Swan Hill

To access a Support at Home Program

An older person may be eligible if they have care needs and they are:

  • 65 years or older
  • 50 years or older – if they are an Aboriginal or Torres Strait Islander person, or homeless or at risk of becoming homeless. If they are under 65, people in these two categories must also be told about other services that may be available to them before having an aged care assessment.

For people who are eligible, the next step is to have an aged care needs assessment.

There are a few ways to apply for an aged care assessment: 

  • Apply online: You can complete the online application for yourself or on behalf of a family member or friend. You will need your Medicare card to hand. The application takes 10 to 15 minutes.
  • Apply on the phone by calling My Aged Care on 1800 200 422: If you need assistance on the call, there are support options available.
  • Apply in person: If you would prefer to talk to someone face to face, you can make an appointment with an Aged Care Specialist Officer (ACSO) at a Services Australia centre by calling 1800 227 475

Support at Home

When you have been assessed for your Support at Home Program through My Aged Care, please contact us for further information. We can arrange to have your agreement signed and your allocated package in place.
