Shared Maternity Care Program

Accreditation - becoming a GP Shared Maternity Care Affiliate at Bendigo Health. 
To apply for Shared Maternity Care affiliation for the 2020 – 2022 triennium at Bendigo Health you need to complete an online credentialing application form. Please click here for information regarding the process.

Reaccreditation requirements.
Every three years, affiliates will be invited to undertake reaccreditation in order to maintain their accreditation. 

Re-credentialing requirements:

  • Review original application and make any changes eg contact details.

Submit the following

  • Current Practice Accreditation
  • Current Personal Medical Indemnity Insurance
  • And attach current record of CPD Points (last 3 years)
